This form will better allow us to quickly diagnose the problem with your amplifier. Please try to answer these questions as completely and honestly as possible. A technician will contact as soon as possible.

What model is the amplifier?
What is the amplifier's serial number?
Was the amplifier recently installed in your station?
Is this the failure of a working amplifier OR have you recently installed this amplifier and are not sure it functions in your station?
Did you attempt to turn the amplifier on without the cover?
Does the amplifier power up?
If it does power up, how long does it stay on?
Does it blow a fuse AND if so which one?
Does it come out of wait state?
Does it fault in receive, standby or transmit?
If it transmits, what power level does it fault at?
Does it fault on a particular band or does it fault on all bands?
Does the receive signal level change when switching from stand-by to operate?
Is RF output power normal on all bands?
What is your RF driving sources (what is your transceiver)?
What antenna(s) are connected to the amplifier?
Have you tested your amp into a dummy load?
Do you have an antenna tuner in-line, if so what type (make & model)?
Do you have a low pass / band pass filter in-line, if so what type (make and model)?
What is your AC line voltage?
